What to expect during your first visit:

At the time of your first visit, please have completed and bring with you the following forms which can be downloaded under the "Office Forms" tab on this website: Medical History Form, Patient Information Forms (fill out the W/C form if applicable to your care), Authorization to Obtain Medical Records Form, and the Notice of Patient Privacy Practice.

In Addition, we ask that you bring the following to your first appointment:

1. Current and active insurance card
2. Photo ID
3. Any and all x-ray films, MRI films and reports relative to the problems you are being seen for
4. Referring physician information for follow-up purposes
5. Referral or authorization if required by your insurance company
6. List of current medications
7. Any and all paperwork or medical records relating to this problem if you have been treated by another Physician or in the Emergency Department
8. A list of questions or concerns for the physician to address
